myPlace Health was founded in 2021 by mission-aligned healthcare leaders and organizations that are committed to drastically improving health outcomes, quality and experience for vulnerable older adults and frail seniors.
We are a multidisciplinary team of clinicians, operators, technologists, entrepreneurs, and builders who are united around our mission and seek meaningful careers that improve the lives of some of the most vulnerable members of our community.

We specialize in providing value-based, comprehensive care and coverage for older adults with significant needs so they can thrive in the homes they love and in the communities they cherish. Our high-touch, team-based medical care and social services enable our participants to enjoy their life to the fullest as they age in their own preferred place, on their own terms, and where they feel the most comfortable.
We pursue this mission through our myPlace PACE (Program of All-Inclusive Care for the Elderly) model, which provides seamless primary care, integrated health plan coverage, personalized social engagement, and customized services delivered in the participant’s preferred place. We are a steward of the public resources entrusted to us and we hold ourselves accountable for coordinating health care in ways that make it more affordable in addition to providing excellent quality and service.
