myPlace PACE provides the care and support you need to thrive in the home you cherish and the community you love. We accomplish this through a model known as the Program of All-Inclusive Care for the Elderly – or PACE for short.
All interested seniors are first screened by members of the myPlace PACE team to verify eligibility. Once verified, we’ll help you with assembling and submitting an application to enroll in myPlace PACE.

Enrolling in myPlace PACE
Enrollment is voluntary, the application is free, and we’ll guide you every step of the way.
1. You or your caregiver requests information from us (contact us or fill out the form above).
2. The myPlace PACE team will reach out or meet in-person to help determine if you are eligible. The applicant and caregivers are invited to visit our Care Center for a tour.
3. For those who are eligible, we will lead you through the whole process which may include a physical evaluation, review of medical history, and other information.
4. myPlace PACE team will submit an application on your behalf for approval from the State of California, which may take up to 30-60 days for a decision.
5. Once the application is approved, enrollment begins with the participant and their caregivers.

What is the cost of the program?
There are no copays or out-of-pocket expenses for any drug, service, or care your care team approves.
Financial Benefits of myPlace PACE
Services approved by the care team are usually provided at no cost if the participant is eligible for Medi-Cal or both Medicare and Medi-Cal.
Participants are responsible for the cost of unauthorized or out-of-program services.
A participant may be required to pay a premium if they are eligible for Medicare and not Medicaid, or if they are not eligible for Medicare or Medicaid benefits.
